UIL Activity Physical Examinations 2021-2022
The UIL Pre-Participation Physical Evaluation (PPE) requires that all students participating in marching band, athletic activities, or both marching band and athletics, must complete the PPE after May 1 for the 2021-2022 school year.
KISD students grades 7-12 who are participating in athletic activities and/or marching band will be required to have a physical from their doctor or family practice physician. Students that want to participate in summer strength and conditioning camp must have a current physical on file as well.
Though Katy ISD recommends that you visit your family doctor for your student’s physical, the following listings are for mass screenings that are available for the economic convenience of our Katy ISD families and students. These examinations will be offered at the nominal fee of $25.

High School students will need to register at their campus. Junior High students will have to register at their feeder high school.
Note: The UIL physical form is the only physical form that will be accepted.
